Finally, I offer bariatric assessment to assess readiness for bariatric surgery. Bariatric assessment timing varies, but will take an average of 3 hours (not including the time it will take me to write up a report and give a feedback session).  My sliding scale for bariatric assessment ends at a household income of $120,000 per year. Anyone above that income scale will have to pay the full $330 self-pay rate. More information on superbills and payment options are on the billing page.

Income Below $30,000/year: $150 per 3 hour session

Income Between $30,000–$60,000/year: $180 per 3 hour session

Income Between $60,000–$90,000/year: $225 per session

Income between $90,000-$120,000 : $ 270 per 3 hour session

Income $120,000 + / year: $330 per 3 hour session session
