I offer bariatric assessment to assess readiness for bariatric surgery.  

I currently accept Aetna insurance. If you are an Aetna member, the amount billed to insurance varies based on the scope of services and copay will be determined by your insurance plan (please reach out to your provider if you want to know what the copay will be for your specific insurance plan).

If you are not using Aetna, I offer a sliding scale for clients

Sliding Scale Bariatric  Assessment

Income Below $30,000/year: $150 per 3 hour session

Income Between $30,000–$60,000/year: $180 per 3 hour session

Income Between $60,000–$90,000/year: $225 per session

Income between $90,000-$120,000 : $ 270 per 3 hour session

Income $120,000 + / year: $330 per 3 hour session session
